Job must be reported within 48 hours prior to commencement.  It is particularly important to include floor, room and apartment number.  Jobs or institutional accounts covering more than one location requires filing for Each Specific Location.  Late filing or incomplete information is a violation of the collective Bargaining Agreement.  Every employer shall report to DC711 in writing within 24 hours loss of any building to Non-Union Employers.  County must be filled in on this form!!!
